Easterseals PORT Health

Comprehensive outpatient and community based support for individuals with developmental disabilities, substance use disorders, and mental health conditions, including individual and group therapy, psychiatric evaluations, medication management, mobile crisis management, and intensive outpatient programs (IOP).

Community living services including supervised group homes, supported apartments, in-home skill building, respite care, and daily living assistance for those with intellectual or developmental disabilities.
